First of all, it's not safe. Losing that amount of weight in that time would be asking for serious trouble. You'd be very sick afterwards.
What diets like this do is make you take in low calories for 4 days usually in the form of juices, vitamins and fibre and you wind up losing weight. Most of this is water weight which you will gain right back after you stop the diet. Then you lose lean muscle mass because you're not taking in any protein. Your body starts to eat itself in order to stay alive. Finally you might lose a pound or two of actual fat but chances are you won't.
In the end you wind up losing maybe 10-12 pounds and a notch on your belt but it comes right back after you stop the diet so you're really not losing anything.
The best way to lose weight is to eat right and exercise. That's it. If you follow this you can safely lose 2-3 pounds a week. When you start losing more, then you're asking for trouble.
